Your privacy is important to us. This Notice of Privacy Practices explains how your protected health information may be used and disclosed, your rights regarding that information, and our responsibilities for maintaining its privacy.

Notice of Privacy Practices

Campagna Therapy, LLC | Effective date: November 1, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice applies to protected health information (PHI) maintained by Campagna Therapy, LLC in connection with its health care services. The practice is required by applicable law to maintain the privacy and security of PHI, provide this notice, and follow its terms. Additional federal or Florida protections may apply to particular information.

Your rights

Access and copies. You may request to inspect or obtain a copy of your health record and other PHI in a designated record set, including an electronic copy when applicable. We may charge a reasonable, cost-based fee as permitted by law and will respond within applicable legal time limits. Certain limited exceptions apply.

Corrections. You may ask us to amend PHI you believe is incorrect or incomplete. We may deny a request for legally permitted reasons and will explain the denial in writing.

Confidential communications. You may ask us to contact you in a particular way or at an alternative address. We will accommodate reasonable requests.

Restrictions. You may request limits on certain uses or disclosures. We are not generally required to agree, except that when you pay in full out of pocket for a health care item or service and request that related information not be disclosed to a health plan for payment or operations, we will honor the request unless disclosure is required by law.

Accounting of disclosures. You may request an accounting of certain disclosures of your PHI, subject to legal exceptions and time limits.

Paper copy and representative. You may obtain a paper copy of this notice even if you agreed to receive it electronically. A legally authorized personal representative may exercise rights on your behalf, subject to applicable law.

Complaints. You may complain to us or to the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights were violated. We will not retaliate against you for filing a complaint.

How we may use and disclose your information

Treatment. We may use and share PHI to provide, coordinate, or manage your care, including communicating with other treating professionals when permitted by law.

Payment. We may use and disclose PHI to obtain payment, determine eligibility or benefits, and process claims, subject to applicable restrictions and law.

Health care operations. We may use and disclose PHI for permitted practice operations such as quality improvement, compliance, audits, credentialing, and administration.

Other disclosures permitted or required by law. Depending on the circumstances and applicable law, we may disclose information for public health or safety, legally required abuse or neglect reporting, health oversight, judicial or administrative proceedings, law enforcement, workers’ compensation, certain government functions, or to prevent or lessen a serious and imminent threat to health or safety. We disclose only as authorized or required by law; a request or subpoena does not automatically authorize release of all information.

People involved in your care. When permitted by law, we may share relevant information with a family member or other person involved in your care or payment, consistent with your preferences and professional judgment. Special rules apply to minors, guardians, and legally authorized representatives.

Business associates. We may share PHI with vendors performing services on our behalf when permitted and with required contractual privacy safeguards.

Uses and disclosures requiring your authorization

We will obtain your written authorization when required by law, including generally for most uses or disclosures of separately maintained psychotherapy notes (as HIPAA defines them), uses or disclosures for marketing that require authorization, and sale of PHI. You may revoke an authorization in writing, except to the extent we have already acted on it or as otherwise provided by law.

Psychotherapy notes are distinct from routine progress notes, diagnoses, treatment plans, and other information in the medical record. Additional protections may apply to certain behavioral health information under Florida law.

Substance use disorder information

Certain substance use disorder treatment records may be subject to the additional protections of 42 CFR Part 2. Where those rules apply, we will comply with their restrictions, consent requirements, and applicable redisclosure rules. Records protected by Part 2 may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your specific consent or a qualifying court order as required by law. If the practice maintains Part 2 records, additional required notice terms and procedures must be incorporated before this notice is issued.

Our responsibilities

We are required to protect the privacy and security of PHI, limit access and disclosure as applicable law requires, and notify affected individuals following a breach of unsecured PHI when legally required. We will make a revised notice available if our privacy practices materially change. We will not retaliate against anyone for exercising privacy rights or making a complaint.
